FACEhyb is not just an idea for an app. It is a care coordination ecosystem in development.
The starting point is simple: families are already doing the work of care coordination, but they are often doing it through scattered messages, memory, emotional labour and fragmented information.
FACEhyb is being built around the network of people surrounding a person, including relatives, friends, community support, care partners and, where relevant, employers supporting working carers.
Where FACEhyb is today
FACEhyb is founder-led and in responsible discovery, design and prototype development. The work is being shaped through lived experience, family insight, care and community conversations, safeguarding, accessibility, privacy and evidence-led testing.
What FACEhyb is building towards
A secure, person-centred coordination layer that helps families and trusted networks understand what is changing, prepare for conversations and decisions, coordinate the practical reality of care, and know when to seek appropriate professional support.
What FACEhyb is not
FACEhyb is not a live care service, clinical system, NHS App replacement, diagnosis tool or emergency route. The website exists to explain the ambition, share the evidence base, invite relevant conversations and build carefully.
